Purpose:
Vascular dysregulation plays a pivotal role in the pathogenesis of primary open angle glaucoma (POAG). However, the effect of POAG on vascular function is unknown .Therefore, in this study we evaluate association of macrovascular arterial stiffness with POAG and controls and the severity of glaucoma
Methods:
An observational cross-sectional study was conducted on 43 POAG patients (34 POAG and 9 NTG) and 30 healthy normal age- matched controls, whom had no medical history of cardiovascular disease(CVD) or CVD risk factors. The ocular parameters including intraocular pressure and vertical cup disc ratio were obtained. Blood pressure, serum cholesterol and serum glucose were also included. The severity of glaucoma were determined at the recruitment period based on the Advanced Glaucoma Intervention Study (AGIS) scoring on Humphrey Visual Field Analysis. Carotid-femoral Pulse Wave Velocity (PWV) was measured using Sphygmocor (AtCor, Medical) to determine the aortic stiffness.
Results:
PWV values differed significantly in POAG patients (15.62 ± 3.95 m/s) compared with controls (12.09 ± 2.78 m/s)(p<0.001) and the result was also significant after adjusted for age (p=0.021). There was no significant difference in blood pressure, serum cholesterol and glucose between glaucoma and controls. There was no significant difference between median of PWV according to the severity group (p=0.103).
Conclusion:
This preliminary outcome indicate that POAG may be a systematic disorder that affects vascular function. However, there is no significant role in determination of severity of glaucoma. A larger sample size is required to determine the role of macrovascular arterial stiffness in POAG.
