Purpose:
|
To evaluate the effects of intravitreal autologous plasmin injection (IVAP) on vitreoretinal diseases and vitreolysis.

Methods:
|
In this interventional, prospective, case series pilot study, 9 eyes were assigned to IVAP. One patient with intraocular lens drop prepared for vitrectomy, another one with vitreomacular traction disease, and the other seven had diabetic retinopathy with clinically significant macular edema. Plasminogen was centriguged from the patients’ plasma. By adding urokinase, plasminogen was converted to the plasmin. A single intravitreal injection of 0.2 ml extracted plasmin was performed. Posterior vitreous detachment (PVD) and potential injection-related complications at week 4 were the primary outcome measure. Secondary outcomes included changes in best-corrected VA in LogMAR, central macular thickness (CMT).

Results:
|
Mean age of the patients was 54.35 years. Four patients were men. Two patients had complete PVD and three patients had partial PVD. Four out of seven patients had decrease in CMT. VA was not changed in six patients, improved in two patients, and decreased n one patient. No uveitis, endophthalmitis, or post injection vitreous hemorrhage was observed.

Conclusion:

This pilot study demonstrated the efficacy of IVP injection on releasing vitreomacular traction and induction of vitreolysis.
