Purpose:

To assess the efficacy of topical diclofenac 0.1% in controlling pain after photorefractive keratectomy (PRK).

Methods:

In a randomized double-masked clinical trial, 68 individuals (age range, 18 to 35 years) having bilateral PRK for myopic correction with or without astigmatism were included. All patients received 100 mg of extended-release oral diclofenac two times (12 hours interval) before the PRK surgery. Post-operatively, patients were randomized into the case and control groups. Case group was administered the diclofenac 0.1% drop, whereas, control group did not. The level of pain, photophobia and functional activity was evaluated by patient after operation.

Results:

All 68 individuals were completed the experiment and included in the statistical analyses. Two days after surgery, no statistically significant difference in pain level was found between both groups. In addition, photophobia and functional activity was not different between the case and control groups. Eyelid edema and conjunctival injection levels were significantly higher in control group (P<0.001).

Conclusion:

Administration of topical diclofenac to a standardized postoperative pain regime did not alleviate post PRK-pain, although it is effective in local signs like eyelid edema and conjunctival injection. The preoperative extended-release oral diclofenac can be recommended as an effective and safe therapy for post-PRK pain.
