Purpose:

To evaluate the efficacy of prophylactic administration of the topical dciclofenac 0.5% on cystoid macular edema (CME) in diabetic patients following uncomplicated phacoemulsification cataract surgery.

Methods:

This prospective randomized controlled trial included 108 eyes (patients) having routine cataract surgery. Fifty four eyes received the conventional postoperative care with steroid drops, whereas the other group (54 eyes) was given a preoperative diclofenac drop four times daily and continued six weeks after surgery. All patients were evaluated by Optical coherence tomography preoperatively and at days 1, 30, and 90.The outcome measures including best corrected visual acuity (BCVA) and OCT findings were compared in and between both groups.

Results:

Mean CMT changed from 235 ± 17and 239 ±16 in baseline to 254 ± 28 and 249 ± 15 at 3 months in the control and case groups respectively( p= 0.003 ). Mean total volume of macula changed from 7.56 ± 0.3 and 7.61 ± 0.22 at baseline to7.65 ± 0.31 and 7.64 ± 0.22 at 3 months in the control and case groups respectively ( p= 0.027 ) .

Conclusion:

This study showed there is a beneficial effect of topical diclofenac for CME prevention following phacoemulsification in diabetic patients.
