Purpose:
|
To evaluate the efficacy of tightening of lateral portion of orbicularis oculi muscle in patients with functional
nasolacrimal duct obstruction (FNLDO).
|
Methods:
|
Operation was performed under local anesthesia from a subciliary incision at the third lateral portion of the lower lid.
Preseptal and pretarsal portions of orbicularis muscle were identified, dissected, tightened by resection of a small portion
of the muscle and secured to the periosteum of lateral orbital rim. Each subject completed a questionnaire about the
amount of epiphora, epiphora effect on daily life, pain, discharge and swelling of the eyes before surgery and one month,
three months and six months after surgery. Furthermore, fluorescein dye disappearance test was performed before surgery
and repeated in the first, third and sixth months following operation. At last visit, the pre- and post-operative data was
analyzed and compared.

Results:

Fifteen eyes of 12 patients (8 males and 4 females) with the mean age of 67.3 years included in the study. Severity of
epiphora, tearing effect on daily life and amount of eye discharge were all significantly decreased following
operation.Fluorescein dye disappearance test result was decreased significantly after one month follow-up in all eyes (15
eyes), at three months follow up in 80% of eyes (12 eyes) and at the end of six months follow up in 66.7% of eyes (10
eyes).No significant complications or side-effects were observed.

Conclusion:
|
In conclusion it seems that tightening of lateral portion of preseptal and pretarsal orbicularis muscle is an effective, safe,
rapid and easy procedure to decrease epiphora in patients with FNDO.
