Purpose:
|
To evaluate a technique of bleb needling for the treatment of persistent early wound leakage after trabeculectomy
|

Methods:
|
In this prospective interventional case series, 24 eyes from 24 patients with persistent early bleb leakage were included. Office-based needling in sterile condition without antifibrotic agents was done to redirect aqueous into a newly formed bleb. Intraocular pressure, bleb leakage and morphology and early complication were assessed.

Results:
|
The mean interval time between filtering surgery and detection of bleb leakage was 7.58 ± 4.43 days and the mean interval time between filtering surgery and needle bleb revision was 19.87 ± 6.74 days. The mean baseline IOP was 7.92 ± 2.46 mm Hg that increased to 11.75 ± 2.84 mm Hg on post-needling day 1 (p < 0.001). Success was achieved in 18 (75%) cases and 17 cases had functional filtering bleb at first month of follow up. All complications, including subconjunctival hemorrhage (25%), leakage from the needle track (13%) and Hyphema (13%) were managed medically.

Conclusion:

needle bleb revision can be considered as an effective and safe option in management of persistent wound leakage after trabeculectomy especially in presence of early bleb scarring.
