Purpose:
|
To evaluate the clinical outcomes of early/prophylactic pars plana vitrectomy performed early in the course of Acute Retinal Necrosis

Methods:
|
Eight patients with the clinical diagnosis of ARN were considered for pars plana vitrectomy. None had any retinal break or detachment.Silicone oil was injected in all patients and boundaries of the retinal necrosis were demarcated by laser photocoagulation in all patients. Patients were under systemic antiviral therapy. Systemic steroid was administered after 24-48 hours of antiviral therapy and all the patients received Aspirin.

Results:

We had follow up period of 11-15 months in 5 patients, 2 months in 1, and 1 month, in 2 patients. None of the five patients with longer follow up developed any retinal break or detachment. The patient with 2 months follow up, developed membrane formation and nasal retinal detachment after a month and second procedure and membranectomy was performed. The other two patients had no complication in short term and are under observation.

Conclusion:

Pars plana vitrectomy can be considered a prophylactic procedure to prevent retinal detachment in ARN. Clearance of dense inflammatory vitreous reaction provides a better visualization to diagnose any developing retinal detachment; this also seems to reduce the risk of subsequent detachment .
