Purpose:
|
To present a modified pressure patching technique for the treatment of a case of chronic hypotonic maculopathy.
|
Methods:
|
We describe a case of long-standing hypotonic maculopathy following trabeculectomy augmented by mitomycin-C in a young, high myopic woman. Surgical bleb revision was unsuccessful in the treatment of the overfiltration; hence, we tried a modified pressure patching technique to alleviate the over filtration and hypotonic maculopathy. In this technique, we applied pressure directly over filtering bleb by pressure patching, while allowing a small palpebral fissure to remain open.

Results:
|
After applying the pressure patch for two weeks, the intraocular pressure rose gradually and hypotonic retinal folds disappeared. With a follow-up period of 20 months, the condition remained stable with excellent clinical and visual results.

Conclusion:
|
Conservative measures, such as modified pressure patching, could be tried for the management of over filtering bleb and hypotonic maculopathy.
