To evaluate sensitivity and specificity of slit light with 90D lens in the diagnosis of cyclotorsion

This prospective masked observational study included 120 eyes of 60 consecutive subjects in two groups(normal and torsion) has been done at Khatam al anbia eye hospital. Evaluations includ: BCVA, cyclo and manifest refraction, prism cover –test, duction and version. subjective test to assess torsion was double Maddox rod, and objective torsion was assessed with indirect ophthalmoscope,fundus photograph (50EX TOPCON)and was diagnosed by two masked investigators using slit lamp biomicroscope with 90D lens. Sensitivity and specificity of slit lamp and indirect ophthalmoscope methods were calculated compare with Fundus photograph as the gold standard test

Contingency coefficient between two examiner with slit lamp method was 68.7%and between two techniques were calculated 61% by the first examiner and 63% by the second.The amounts of sensitivity and specificity of indirect ophthalmoscope method in the diagnosis of extorsion were 50% ,92% and in the diagnosis of intorsion were 60%,93%.Contingency coefficient between these methods was 60%

The fundus photographic technique remains the gold standard test in the diagnosis of ocular torsion,but slit lamp and 90D lens method can be used as the replacement of indirect ophthalmoscope in the diagnosis of cyclotorsion .
