Purpose:

to compare the incidence of post operative endophthalmitis without and with different doses of intracameral cefazolin injection during phacoemulsification
|
Methods:
|
Group 1 comprised 848 eye of 730 patients who had phacoemulsification between November 2009 and November 2010 without intracameral cefazolin injection. Group 2 consisted 1098 eye of 660 patients operated between December 2010 and December 2011 with 3mg/0.1ml intracameral cefazolin injection in conclusion of operation. Group 3 comprised 1182 eye of 710 patients who had phacoemulsification between January 2012 and January 2013 with 6mg /0.2ml intracameral cefazolin injection at the end of phacoemulsification.

Results:
|
one case had endophthalmitis in group 2 as compared to 5 in group 1 and there was no endophthalmitis in group 3. In no cefazolin injection group (group 1) 2 out of 5 had posterior capsule rupture intraoperatively. 2 had chronic blepharitis and 4 had diabetes mellitus. In group 2 the endophthalmitis case was poor control diabetes mellitus but the operation was uneventful.

Conclusion:
|
intracameral cefazolin injection at conclusion of operation caused decrease in endophthalmitis rate and 6mg/0.2ml cefazolin was much better in preventing of infection without any special side effect thereafter. Factors like chronic blepharitis, posterior capsule rupture and diabetes mellitus increase chances of endophthalmitis postoperatively.
