Purpose:

Case report Title: Bilateral posterior polar cataract with spontaneous partial dissolution of lens material

Methods:

Introduction: Posterior polar cataract is a rare form of congenital cataract. The predisposition of posterior polar cataract to posterior capsule tear during phacoemulsification surgery is well recognized.

Results:

Case presentation: We report a 61-year-old woman with history of blurred vision from 3 month ago in both eyes. There was no pain, floater, photopsia. She denied trauma or previous eye problems .The visual acuity in the right eye was hand movements and 1 meter counting finger in the left eye that not improving with pinhole. The slitlamp examination showed a posterior polar cataract and marked loss of anterior lens convexity in the both eyes, and there was dense cortical material in the capsular bag, that prevented visualization of the fundus, but B-scan ultrasonography revealed a normal retina and vitreus. During ECCE surgery we found an adhesion between anterior and posterior capsule .The small nuclear matterial was removed, along with the lens cortex. One PMMA lens placed in sulcus in left eye with an intact posterior capsule. In right eye the patient was left aphakic, because posterior capsule was not intact. She was scheduled to have secondary intraocular lens implantation at a later date.

Conclusion:

Conclusion: We believe this is the first report of an unusual form in bilateral posterior polar cataract with spontaneous partial dissolution of lens material.
