A 62-year-old Chinese man with neovascular glaucoma presented with sudden reduction of vision in the left eye after Bimatoprost instillation. The hyphema most likely originated from the iris new vessels as a result of a lowering of intraocular pressure secondary to Bimatoprost instillation.

Conclusion:
|
the reduction of IOP with bimatoprost may have developed to hyphema. The new vessels in the angle and prove to bleeding especially if there is a sudden decrease in IOP. The presence of the anterior chamber IOL may have contributes to the hyphema. Micro movements of the anterior chamber IOL may occur with eye movement or eye rubbing.
